WELCOME TO YOUR QUESTIONNAIRE: This is designed to give us a good understanding of you, your challenges and your environment. There are 9 sections to complete:

- How is your body feeling

- Digestion
Based on your answers we will be able to give you an overview of how well you are. The information is meant to give you guidance and it’s not a diagnosis of any condition you may have or replace the advice/guidance from a medical or health professional.
WHAT HAPPENS NEXT: After you have submitted your answers the Metanoia Wellness team will review them and alongside the 1-2-1 assessment we will write a report which we will send you via email with guidance that is personalised to you. Because this is a personal email to you, we take a few days to come back. We will discuss this report in our session together.
HOW TO GET THE BEST OUT OF THIS EXPERIENCE:
– It will take approximately 20 minutes to complete, so make sure you take time for this and you can be undisturbed so you have the possibility to think about your answers;
-We would highly recommend completing this questionnaire on a desktop to get the best experience. However, if you wish you can complete this on mobile.
– We care not only about how you feel but also we care about your privacy. So rest assured that your details are confidential and encrypted and we will not share any personal data with anyone. We collect the data to understand and assess your wellbeing. We will only use the email with the purpose to send you back the results and guidance.
